The Service Request Form provides RTE technicians with a detailed report of the condition and problem of the part needing repair. Complete and submit one Service Request Form for each part sent to us for repair. Please provide as much information as possible, as the information you provide can speed up the diagnostics and result in faster turnaround times for repair.
